Applying to the Year 2008 PSU-MHIRT program

Instructions for applying by mail:

  1. Download the application in MS Word format or PDF format.
  2. Download the application instructions in MS Word format or PDF format.
  3. Download the Faculty Recommendation Form in MS Word format or PDF format.
  4. Download the announcement in MS Word format or PDF format.
  5. Print the announcement, application, application instructions and two copies of the Faculty Recommendation Form.
  6. Follow the application instructions to complete the application.
  7. Mail or fax all materials to:

    We must receive ALL materials, including the transcript(s) of your college work and recommendations from two faculty members, by November 30, 2007!
